You can add a surname to all those documents. Each authority has a defined process and accepts supporting evidence. You can get a single verifiable identity document showing the new surname first through affidavit or newspaper or gazette publication then, update the Aadhaar Card, PAN card, Voter ID, Passport. Aadhaar is the de-facto identity card and is widely accepted. PAN and Voter ID are linked to Aadhaar on many portals and Passport is the primary identity for travel and university admissions. Having a Gazette notification, affidavit and government photo ID makes all the other updates easier.
To create a document showing the new surname, prepare a sworn affidavit stating your details, the reasoning behind addition to the name and that the same person is being referred. You can get it notarised. Publish a short ad in two local newspapers stating the name change. Now, submit the affidavit, newspaper clipping and ID proof to the Department of Publication for publication in the Gazette, since this is accepted as a primary proof of name change. After this, visit an Aadhaar Enrolment or Update Centre. Carry the above mentioned documents and the current Aadhaar. For updating PAN, use the PAN correction webform on wither NSDL or UTIITSL portal and submit proof of identity showing the new name. You will be issued a new PAN card. For updated Voter ID, apply through NVSP. Upload supporting documents, the BLO verifies and will update you on the roll. If you require reissued passport, you must apply for the name change on Passport Seva. Herein, police verification may be required. For correct in educational institution documents, CBSE has a formal process and forms of correction or name change. Submit the application with affidavit, gazette and school forwarding letter.
