1. Apply to the Registrar of Births & Deaths of the area where birth was originally registered.
2. Submit:
Original birth certificate,
Documentary proof of correct DOB (hospital records, school records, affidavit, etc.),
Application for correction.
3. Registrar may call for inquiry or supporting documents.
Some states allow online correction applications through their municipal/corporation website, but often you still need to visit the office for verification. But generally you need to visit the office and provide an affidavit as well.
